Our business manager will assist you to pre-authorize your periodontal and dental implant procedures. We are a Delta Dental Premier Provider, but also accept most insurance plans. Please provide us with recent insurance information, social security numbers and date of birth for you and your spouse. This will speed up the response from your insurance company.
All procedures not covered by your insurance plan will be charged at our usual/customary fees. The fee will be given to you prior to doing a procedure.
